What would be a good workout plan in order to lose weight and gain more overall fitness? I am just getting ready to start, and I want to be able to start slow and build up without becoming frustrated and quitting. Any ideas?

Before you decide on a workout plan you need to solve the other problems that are hindering you. The majority of weight loss is determined by diet and portion control. You are absolutely correct in your assessment that you need to quit smoking, fast food, sweets, and all of that other stuff. The hard part is doing it. And sticking to it. You need to learn self control and walk past the free donut box. Not eating that pastry is its own reward. I would personally suggest that you continue with pushups and biking. The issue with your knee will be a major issue but there are workarounds. Do you have access to a swimming pool? Swimming would likely be a good low impact exercise for you. Seriously, quit smoking. Quit fast food. Quit candy and sweets. You know better. Now act on it.
